Undiagnosed pleural effusion, in staging Thoracoscopy is an age-old procedure, valuable in the evaluation of any pleural pathologies. It has again currently gained much importance, since it not only helps in providing accurate diagnosis, but also has therapeutic potential in the form of pleurodesis and adhesiolysis. The main uses of this procedure is in the evaluation of of lung cancer and in the evaluation of Pleural malignancy. Another advantage is the use of talc insufflation procedure intrapleurally in case of recurrent pneumothorax and malignant pleural effusion to prevent recurrence in these conditions. This has helped a lot in improving the quality of life in the terminally ill patients. This procedure can be done under local anesthesia and conscious sedation. Lately bullae excision in the lungs, mediastinal lymph node biopsy and pericardial window creation for pericardial effusions can be done with the medical thoracoscopy.
